How to Register with the Surgery

If you reside within our practice area and are not registered with another Doctor locally, then you will be able to register and will need to contact the surgery on 01234 266866 to arrange registration. If you do have a doctor locally but would like to register at the practice please put your request in writing addressed to Mrs Von Balodis – Practice Manager.

Completed forms should be returned to the surgery with identification.
